Car Rider Pick Up Form
This form should be filled out by families who plan to pick up their child from school more than 1 time in a month.  These pickups are REGULAR (every day, every other week, every Tuesday, when with a certain parent etc.).  This is not a form for OCCASIONAL pick-ups (random dentist appt, doctor appointments, vacations etc.)

If you should find that your needs change and your child will be a REGULAR car rider (more than 1 time a month) you can fill out this form at any time.

After your form is complete, a Scotland staff member will get back to you with your car rider number.  You WILL receive a paper number upon your first pick-up of your child.  If you (and your child) know your pick-up number on the FIRST day, it will expedite our process.

Please have ID ready to present at any time you (or another person) is picking up your child.
