Keep in mind the registration process is considered complete when the registration form, payment (minimum $40.00 processing fee**), and Baptismal certificate are submitted. Please provide a copy of Baptism Certificates for all children/teens in all programs. You may scan and email (Tere@padreserra.org) the certificate or mail in a hard copy to Faith Formation 5205 Upland Rd. Camarillo CA 93012.
Completed registrations received, are time stamped with an official priority number. That number drives group placement until groups are full. So please complete and submit your information as soon as possible for the best selection.
After you submit this form, you have the option to make a payment via PayPal (you can checkout without a PayPal account) or mail in your check payable to PADRE SERRA PARISH.
