The undersigned appoints Ann Castaldo, Secretary of the Four Mile Lake Association, on behalf of the Board of Directors, as my proxyholder to attend the Annual General Meeting for the 2021/22 Fiscal Year to be held on Sunday, July 10, 2022 at the Burnt River Community Centre.
