This permission slip gives your youth the permission to participate in virtual and/or in person programming.
All youth must have a permission slip on file to attend programming.

In Person Programming

Youth will come together to play group games, participate in a fun science activity & have free time on the electronics. (Computers, video games, non contact recreational games.)

Virtual Programming

Youth will come together via zoom and play online group games and participate in a fun art activity.
This option gives youth the chance to interact with their peers from home.

Please read the following statement of COVID19 safety policies and procedures and type your name below to sign & confirm you and your youth's understanding.

•Both staff and youth will be expected to maintain a 6 foot distance with other members.

•Both staff and youth will be expected to wear masks during their time at MYC except during snack in which all youth will be seated 6-8 feet apart..

•Youth and staff will have their temperature taken and answer health screening questions at arrival each day, if their temperature is above 100F they will be asked to leave until tested for COVID19.

•Parent's and youth agree that youth will not come to MYC on days they are feeling ill.

•Youth members will be expected to follow COVID19 safety policies and procedures. If a youth is unable to do so a parent will be contacted and youth will be asked to take a break from MYC activities.

•Parent's will contact MYC immediately if a youth member tests positive for COVID-19 (MYC will NOT share names or personal information regarding a positive COVID19 result, but will implement contact tracing procedures).
