ALL information must be completed in your application and verifiable to be considered complete.  Any missing documentation may result in your application not being considered.  Information will NOT be accepted via paper form, fax,  or email--Applications must be submitted ONLINE via this form.  

**Official Transcripts from other Colleges or Universities and ACT Scores must be delivered to the Admissions office on or before the application deadline of May 10, 2021.  It is YOUR responsibility to ensure the Transcripts have been received.  

Information WILL NOT be released from applications once received.  You will be notified by email of your status regarding Phase 2 of the application process.  No information regarding individual admission status will be given via telephone or email; do not call Admissions or the Program to obtain your status.
