About the Spotlight Program:
The CDOC Spotlight program is designed to introduce and focus attention on cancer plan projects or members. We encourage you to fill out the Spotlight Questionnaire below, provide a few details about your organization, be as thorough as possible, and provide example where applicable. While we can’t guarantee that everyone will be spotlighted, we will endeavor to spotlight as many as possible over time.

By submitting this form, you acknowledge that CDOC will share this information and this photo on https://www.cdoconline.net and/or other platforms operated by CDOC. You further acknowledge that the CDOC reserves the right to use all or several of your questionnaire responses and to edit your responses consistent with the CDOC publication. Your telephone and e-mail information will not be included in the post, unless you make that request.
