Thanks so much for joining Horizon PTA. No commitment or volunteering is required to be a PTA member, you’re simply showing your support for our great students, school, parents and staff. The vision of your Horizon Elementary PTA is to every child's potential a reality by engaging and empowering families and communities to advocate for all children.

This form must be completed in order to validate your PTA membership. Payment can be made online through our Square Shop (link provided at the end of this form), or by cash or check (payable to Horizon PTA) in the PTA box located in the Horizon Office.
