To attend CanUUdle and all CUC youth events, each adult advisor/ sponsor who is going to stay on site at the event must have a Congregational Involvement form signed by a church leader or staff member. This form affirms that the adult volunteer has an established relationship with their local UU community and has been through a vetting process.  You may use this form to submit congregational involvements for all of the adult volunteers from your congregation who are attending the youth event.

People who may vouch for an adult's involvement may include but are not limited to:
a Director of Religious Education (or equivalent staff), a Minister, other staff member, or a Congregational Board member.

Family members should not sign this form for each other.
