I hereby consent to participate in the research project titled "Collecting Data On General Osteopathic Treatment Efficacy At Summit Osteopathy."

I understand that the purpose of this research project is to investigate the effectiveness of osteopathic treatment provided at Summit Osteopathy and to assess improvements in pain levels and daily functioning among new patients over a period of 90 days.

I understand that my participation in this research project is entirely voluntary, and I have the right to withdraw at any stage without facing any consequences or penalties.

I understand that if I choose to participate, I will be required to complete two online surveys: one shortly before my initial session at Summit Osteopathy and another three months later.

I understand that my responses to the surveys will be kept strictly confidential and will be used solely for research purposes. My data will be stored securely and anonymously, in compliance with the Privacy Act of Australia 1988.

I understand that there is no financial compensation for participating in this research project.

I understand that my participation in this research project will not affect my experience (clinical or otherwise) at Summit Osteopathy, and there are no disadvantages or adverse consequences for not participating or for withdrawing from the research.

I understand that I must sign and submit this consent form to participate in the research project.
