Please note: Membership in ArtForms is required. If your membership has lapsed or is due before March 1, 2021 you must renew your membership when you send in this application form. While not required, artists are encouraged to submit a short biography and/or an artist statement at the time of application. Entry fee and dues may be combined in one payment.
