By submitting this form I accept that, although CDN Trust will endeavour to exercise all due care, they will not be liable for any injury sustained by my child while in the care of CDN Trust nor for any loss or damage suffered to any property in my child’s possession. In the case of an emergency I understand that my child will be responsible for their own property and behaviour while part of this programme.

The above information is true and correct. I authorise CDN Trust to take action on my behalf for my child in case of sudden illness or injury and they will attempt contact with a contact person supplied above. I agree to abide by CDN Trust policies.

CDN Trust wishes to collect the personal details for the purposes of program registration and evaluation. You have the right to access and check any details you have supplied. All the information gathered will be used in accordance with the Privacy Act 1993.

CDN Trust takes audio and visual images of campers while on camp for promotional and funding purposes. It is the Trust’s policy that any photos for publication are positive depictions of the campers and will be used in such a way to preserve anonymity. Please advise CDN Trust if you have any concerns about the publication of the nominated camper’s photo.

Complaints Procedure:
- Raise the concern, suggestion or complaint with the programme supervisor.
- If this is not appropriate discuss your concern, suggestion or complaint with the CDN Trust Manager or Chairperson of the Board Ph 942-3434.
- Alternatively put your concerns in writing Attention to The Chairperson, CDN Trust, 44 Elizabeth St. Riccarton, Christchurch 8041.

Programme & Safety Details:
- Staff and volunteers are trained in health and safety and leadership skills.
- This camp is for young people who are in years 9, 10 or 11 only unless express permission is given by the Camp Director
- Staff to child ratio is max 1:10 (Leader/children) for activities at a base, and 1:6 for programme's away from a specific venue.
- We will always have at least one Leader trained in first aid on every programme.
- We are a Christian organisation and we will share some Biblical principles on this programme.
- Please be aware that because safety is our first consideration our policy needs to be strictly adhered to. Parents will be phoned if their children are causing disruption to the programme. Where serious harm may occur the child/children will be removed immediately. Parents must be able to pick up their child from camp if necessary.
- The camp will proceed regardless of the weather, however some activities are weather and availability dependent. If we are forced to change activities due to weather or other circumstances we cannot refund camp fees. Campers must wear proper (safe) clothing for all activities.
- If camp is full, you will be put on a waiting list. Registrations are generally taken on a first come-first serve basis, however we reserve the right to also accept or decline registrations on the basis of special need and specific programme requirements.
