The West Virginia 4-H All Stars annually recognize Outstanding members who have shown exceptional service in their county.

It is requested to have every county recognize an All Star or group of All Stars who are active Life Members and have continued in the All Star tradition of service.

Please note that current WVU Extension employees are not eligible to be nominated as an Outstanding All Star.

The information recorded in this form will be used when recipients are recognized for their hard work and dedication to provide service.
