This form is for LONG BRANCH ELEMENTARY families only.

Parents wishing to make a request for their child for the upcoming school year MUST make the request by completing this form in full. Forms that are not completely filled out will not be considered. Parents making multiple teacher requests for multiple children must complete a form for each child.

Parents are encouraged to remember that just as each child is unique, so is each teacher.

The school administration makes all final classroom placements to ensure an even mixture of students in each class. There are many factors to consider when placing students. The final placement decision is based upon a variety of factors including student academic and social needs, total class size, male/female ratio, etc. No more than 50% of any given class will be made up of parent requests. The administration will do our best to honor parent requests; however, due to other factors we may not be able to honor every request. Please be sure to indicate both a first AND second choice for your child.

Classroom assignments will be shared a few days before school begins and available in the office at open house.

CLASSROOM REQUESTS MUST BE MADE PRIOR THE END OF THE SCHOOL DAY ON MAY 22, 2024 TO BE CONSIDERED.
