We're sending Philip off in style and with gratitude for his extraordinary 37 year ministry amongst us! This is the RSVP form for the celebration. We need to have a clear idea of who is planning to attend the pieces of the celebration so that we can make appropriate plans.

The Evensong at 4 pm is open for all
There will be a (cash bar) cocktail hour from 5- 6 pm
The dinner in Dove Hall from 6-8 pm will be ticketed at $25 per person (children 12 and under, free).

If you (and your party) are coming to the dinner, you are asked to pay $25 per person for everyone over 12 years old. Please indicate the method you will use to pay.
