22-23 Consent for Release of                                  Educational Records / Transcripts
I, parent or guardian, give Vermillion High School consent to release all records that may be requested for the below-mentioned student when a college application for admission is initiated. These records may include the transcripts (including test scores), teacher recommendation, counselor recommendation, and secondary school report. I hereby give additional consent for the same supporting documents to be given to an organization who may want to grant a scholarship to my child or to an athletic recruiter that may be reviewing my child's records for athletic recruiting purposes.
