Complete this form if you would like your K-5th grade learner pulled from class to have their portrait taken. Ordering through Lifetouch does not notify us.

CCMS/CCHS (6th - 12th grades) - will have opportunity to come out of class on their own for photos.
CCEE (Preschool) - does not have a make up day.

Your child can take their photo for any reason (including the following):
* Parent was not satisfied with original photos.
* Child missed portrait day in September.
* Child is a new learner at CCS.
* Child did not wear appropriate school uniform.
* Any other reason.
