Each year, St. Joseph School asks all families to fill out required forms as part of the yearly enrollment process.  Please answer the questions below and submit the form by September 23rd. Thank you!

Novel Coronavirus Acknowledgement and Assumption of Risk:  I acknowledge and understand, the novel coronavirus, COVID-19 has been declared a worldwide pandemic by the World Health Organization. Further, that COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. I also acknowledge, that federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited or limited the congregation of groups of people. I hereby agree, represent and warrant that neither the undersigned nor any registered participant child(ren) shall visit or utilize the facilities, programs, activities, or services of ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT  if he or she (1) experiences symptoms of COVID-19, including, without limitation, fever, cough or shortness of breath, or (2) has suspected or diagnosed/confirmed case of COVID-19. The undersigned agrees to notify (ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT) immediately if any of the foregoing access or use restrictions may apply.(ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT) has put in place preventative measures to reduce the spread of COVID-19.  I agree to comply with measures that ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT may require to best protect against the introduction of viruses at ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT, including, but not limited to, hygiene practices related to myself and/or my child(ren). ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT cannot guarantee that I or my child(ren) will not become infected with COVID-19. I understand and agree that attending ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT could increase my risk and my child(ren)’s risk of contracting COVID-19.By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by participating in programs of or attending ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT may result from the actions, omissions, or negligence of myself and others, including, but not limited to, ST. JOSEPH CHURCH AND SCHOOL - ROSEMOUNT employees, volunteers, and program participants and their families. *
MEDIA CONSENT AND RELEASE: By checking "I agree", I (we) the parent (s)/Guardian(s) of the above child(ren), do hereby consent that St. Joseph School and the Archdiocese of St. Paul and Minneapolis be permitted to use and publish for any lawful purposes, including without limitation for the purpose of publicity, education, advertising, media relations, press coverage, and other promotion, advertising, commercial or publicity purposed, do authorize, and waive any and all rights to approve, the release, publication, dissemination, distribution, display, use and/or reproduction of any and all photos, videos, Student Content and Student Works of or relating to my child in School Publications for  the name and likeness of my child. This authorization and consent permits such use to associate my child’s first name with the likeness for such purposes provided such use is consistent with the acceptable use policy for electronic communications and other policies. The parent/guardian acknowledges having read this release, having had the opportunity to consider and understand its terms and does hereby execute it voluntarily and with knowledge of its significance. *
PARENT HANDBOOK:  I have read and understand that by having my student (s) enrolled in St. Joseph School and Preschool, I will adhere to the policies and procedures outlined in the St. Joseph School handbook and Faithful Beginnings handbook.  This includes, but is not limited to, attendance policies, uniform policies, and the school schedule. Please see link above to read full handbook. *
WALKING PERMISSION SLIP:  I, the parent/guardian, grant permission for my child(ren) listed above to participate in walking field trips within walking distance of St. Joseph School.  Trips may include, but are not limited to, nature hikes, places of interest and parks.  I warrant that my child is in good health.  In consideration of my child’s participation, I agree to indemnify the Church and School of St. Joseph - Rosemount and the Archdiocese of Saint Paul and Minneapolis from any claims or law suits brought against the Church and School of St. Joseph - Rosemount /Archdiocese of Saint Paul and Minneapolis by myself, my child or others, that arises out of any behavior by my child at the event/activity described above.  I also agree to pay reasonable attorney’s fees or expenses incurred by the parish/school and the Archdiocese in defense of such a claim/suit.  This agreement is intended to give the teachers permission to take students on these various walks, and will be kept on file in the school for this school year.   *

Grades K - 8 ONLY: ACETAMINOPHEN PERMISSION:  May the school give this student acetaminophen?  Parents/guardians will always be notified when this occurs.   *
Grades 6 - 8 ONLY: IBUPROFEN PERMISSION:  May the school give this student ibuprofen?  Parents/guardians will always be notified when this occurs. *
