2021 Citizen of the Year Nomination Form
The Citizen of the Year Award is given to a local resident whose service and dedication have made a significant contribution to the community. The award recipient will demonstrate outstanding commitment and dedication to the betterment of the quality of everyday life in Olmsted Falls. The Citizen of the Year will be selected from all of the submitted nominations by the Park and Recreation Board. The award will be presented directly to the recipient by Mayor Graven at the annual Falls Day celebration on Sunday October 3, 2021.
QUALIFICATION CRITERIA:
-Resident of Olmsted Falls for at least 12 months
-Exemplified above average volunteer and/or philanthropic dedication and effort, providing significant value to the community. These efforts may be related to civic organizations, school activities, recreation, church, community events or special volunteer service to the community.
NOTE: It is the intent of the Park and Recreation Board of the City of Olmsted Falls to have the name of the award recipient published and information related to the nomination made public. Although the identity of the award recipient is kept confidential until presentation of the award, it is assumed that the nominee has agreed to stand for nomination and will agree to participate in the award presentation. The recipient must be able to uphold the dignity of the award and be capable of performing some duties if/as called upon.
