All non-PGMHA players who wish to tryout for a U13 (male), U13 Development (female, non-carded), U15 (female) or U18 (female) PGMHA rep. team must complete this form in addition to (1) being pre-registered with your residential home association, (2) registering for PGMHA Rep Tryouts and (3) paying the $150 tryout fee. All 3 items must be completed a minimum of two weeks prior to the commencement of tryouts.
