If you wish to speak to a member of staff about our After School Care provision, please contact the school and ask for Sabrina Thomas. She will be glad to help you access and understand all the information about our rates, terms and conditions, as well as helping you complete this form.

Note that you must complete a separate form for each child you wish to access After School Care.

Once you have completed this form, we will be in touch with you with you to confirm a place for your child.
