Assumption of Risk: I acknowledge that participation in basketball activities involves inherent risks, including but not limited to physical injury, property damage, and other risks associated with physical activity. I am aware of these risks and accept them on behalf of my child.

Medical Authorization: I authorize the camp staff to seek medical attention and treatment for my child in case of injury or illness during camp hours. I understand that I will be responsible for any medical expenses incurred.

Release and Waiver of Liability: I, on behalf of my child and myself, release and hold harmless Klinik, Bob Singh Foundation, and Jumpshot Generosity, its organizers, staff, volunteers, and sponsors from any and all claims, liabilities, demands, actions, or causes of action that may arise as a result of my child's participation in the camp, including but not limited to personal injury, property damage, or any other harm sustained during the camp.

Photography and Publicity: I grant permission for Klinik, Bob Singh Foundation, and Jumpshot Generosity, to use photographs and videos of my child for promotional and publicity purposes. I understand that these materials may be used in various media outlets, including but not limited to websites, social media, and printed materials.

Compliance with Camp Rules: I understand that my child is expected to follow the camp rules and guidelines provided by the camp staff. Failure to comply may result in the removal of my child from camp without a refund.

Code of Conduct: I understand that my child is expected to conduct themselves in a respectful and sportsman like manner towards coaches, fellow campers, and other participants. Any disruptive or inappropriate behaviour may result in the removal of my child from camp without a refund.
