August 17-21 at Chatham Christian School, 475 Keil Drive S, Chatham
Monday-Thursday from 9am-2:30pm
Friday from 10am-1:00pm, at Grace CRC, 265 Tweedsmuir Ave W, with Soccer World Cup Activities followed by a BBQ Lunch.
AFTER June 1, due to concerns related to COVID-19, registration must be confirmed by $25 e-transfer to sc4cchatham@gmail.com, password: Soccer2020 or cash/cheque brought to Grace CRC
Children must be entering grade 3-grade 6 in September to be eligible for Soccer Camp 4 Christ 2020.

Behaviour Expectation Policy: If a child is struggling to participate well in his or her team at soccer camp, his or her team leader will ask one of our designated Head Coaches to assist the child until the issue can be resolved.  If the issue cannot be resolved after 15 minutes of one-to-one assistance with a Head Coach, the Camp Director will contact the child’s parent/guardian and ask that the child be picked up from camp as soon as possible.  The child may attend camp the next day. Friday BBQ Lunch: On August 21 at 10:00am, there will be World Cup Soccer Activities followed by a BBQ Lunch at 11:30am at Grace CRC, 255 Tweedsmuir Ave West.  Friends and family are very welcome!  Please indicate how many persons will be attending the BBQ (including the child registering for camp).  If you are filling out registration forms for more than one child, please include all guests on just one form.
Financial Assistance: I am in need of financial assistance from the churches for the cost of soccer camp

Payment Information: We will again be accepting e-transfers to cover the registration fee. AFTER JUNE 1 (due to COVID-19 concerns), and we have confirmed that there is a spot available for you, please send your e-transfer to sc4cchatham@gmail.com and use the password: Soccer2020  If you cannot use e-transfer, please comment below and bring payment to Grace CRC during office hours.

Medical/Video/Photography Authorization: I hereby authorize the staff and volunteers of the Soccer Camp 4 Christ to make any and all decisions regarding the emergency treatment of my child.  I also hereby authorize the staff of the Soccer Camp 4 Christ to take video and still photos of my child during camp.  I have read, understood and agree with the above and hereby release and discharge all parties associated with this camp from any and all claims, demands, actions, and causes of action, that I/we or my/our child(ren) incur(s).   *

You will be notified that a spot is available for you, or that you are on the waiting list, when you have submitted this form. *This year, due to concerns surrounding COVID-19, we will not request etransfers until June 1 when we can confirm the feasibility of running SC4C.
