REGISTRATION FORM:                                                                                                                                                                    Quarantine Package
This registration form is for existing DACOTA Users and Certified Michigan Corrections Professionals who qualify for FREE enrollment in the Quarantine Package.  Please fill out this form to let us know how many individuals you wish to enroll for 14-day Access to the QPACK.  Once we receive the registration, NIJO will email each registered person with instructions for accessing the package.  If an individual is already enrolled as a DACOTA User, the QUARANTINE PACKAGE Learning Plan will show up in their DACOTA dropdown menu and will be accessible for 14 days from the time of enrollment in the package.  (If you should have any questions, please contact us at online@jailtraining.org)
