Please complete this form to apply for a place at one of our after school clubs.
If your child is allocated a place at a club, you will receive confirmation by Friday 22nd December.
If you do not receive confirmation, unfortunately your child has not been allocated a place at a club.
Places are allocated on a first-come-first-served basis. The sooner you complete this form, the more chance you have of securing a place at a club.
All clubs are free to attend.
All clubs finish at 4:15pm. Parents must collect their child on time via the main entrance on Piers Road.
