Hello! Thank you for using this form to request sick pay with AEGIS Security & Investigations Inc. Please complete this form before the start of the shift you are requesting sick pay for, otherwise there may be a delay in being paid promptly and appropriately. Late requests will be processed on the next available payroll cycle; however late requests are against company protocol. Also, please fill out 1 form per shift date.
Please note, California regulates sick time usage and while you accrue sick time from day 1, you can start using your sick time on day 90 of working with the company, so please check to verify your start date before filling out the form!
Also, California has specific requirements as to what you can use sick time for. Sick time is not the same thing as vacation or PTO. "An employee can take paid leave for employee's own or a family member for the diagnosis, care, or treatment of an existing health condition or preventative care or for specified purposes for an employee who is a victim of domestic violence, sexual assault or stalking."
