Madison Youth Soccer Jamboree
Please use the form below to submit your team of 3 or 4 players for the 3v3 tournament.  Each team will play in a series of 3 on 3 games over 2 hours on Thursday, June 17, 2021 at Salt Meadow.   List the names of the players on your team, along with a team name.  If your player doesn't have a team, you can sign up as a single and we will try to form a team for you.  Players must be in the same age/grade category, but co-ed teams are ok.   Grade categories reference the recently concluded 2020-21 school year.   Only one form is necessary per team.  We are looking forward to a fun day of soccer to kick off the summer!

This should be a fun day and a chance to play together after school ends and before summer vacations.
