This EOTC (Education Outside the Classroom) form covers events which occur during school hours till approximately 9.00pm.
For example: Road Safety Training, visiting nearby Early Childhood Centres, rewards activities outside of school grounds, discos, visiting local businesses, athletics events outside of school grounds and cross country. This form will replace all local activity Student Consent forms for activities within walking distance around our local community, or transport by bus to other educational facilities within our north-west and west area.
Information about any EOTC activity will be notified to parents through our usual communication channels.
Any trip which requires transport utilising personal cars or requires payment, parent consent googleforms will be sought.
It is important that this form is completed at the start of the year or upon enrolment for all students who will participating in EOTC events as described above. Details on this form will remain confidential to Riverhead School unless there is a case of a medical emergency where information will be provided to medical services. It is crucial that you provide us with up to date information which is accurate and complete, to allow us to plan appropriately for EOTC events.
