Re-Enrollment Checklist:
*Students entering kindergarten must turn five years of age on or before September 1, 2020
 Online application
 Explanation of tuition fees form
 Parent handbook acceptance form
 Parent volunteer agreement form
 Copy of updated immunization card
 Copy of Report of Health Examination for School Entry form for kindergarten and first-grade students
 Two volunteer checks $300.00 each post-dated for January 15, 2021, and June 11, 2021. The checks will only be cashed if 15 volunteer hours are not completed by January 15, 2021, and 15 volunteer hours are not completed by June 11, 2021
 Registration fee of $150.00 per student if received by 3:00 pm on February 22, 2020; thereafter, it will be $250.00 per student due at the time of registration (non-refundable)
 Full tuition payment post-dated July 1, 2020, or 10 post-dated checks July 1, 2020– April 1, 2021
