Health Plan Memberships: Your membership could be FREE! Ask your Medicare or Health Insurance Plan if a Y Membership is paid for by your plan. Contact HMSA, Kaiser, UHA, UnitedHealthcare or Ohana for more details!
If you can't make it to our facilities, we have some alternative options for you:

This form is required to complete your cancellation. In order to stop billings, forms need to be returned at least 7 days prior to your next billing date. *Refunds will only be processed for payments processed within the last 7 days of your request.
