1) This form is not valid without a deposit at the time of application and will be discarded within 24 hours.
2) Student/ applicant must be the same person. This form must be completed by the actual person attending the school.
3) Student/ applicant forfeits any priority or benefits associated with priority if this form is submitted without payment or completed by anyone else.  
4) Riders/ Drivers must be truthful and accurate in their responses.
5) Any material misrepresentations on this application will be grounds for expulsion from the BAJA RALLY SCHOOL.
6) Applicants may not participate in BAJA RALLY SCHOOL without the minimum required prerequisites listed on website

7) BAJA RALLY SCHOOL Administrators may exclude any applicant they believe is either a safety hazard to themselves or others or who lacks the basic skills to ride "off road" terrain at an "intermediate level capacity". Any sign of intoxication or drug use will result in suspension or expulsion.
8) Applicants are required to provide their individual or group plan health insurance information or attest they are uninsured at time of application. If you decline to provide this information it is at your own risk. If you are unsure about why you should provide this information, you should contact the school directly by phone to discuss.
9) BAJA RALLY, LLC WILL NOT SHARE YOUR INFORMATION WITH ANYONE EXCEPT AS NEEDED BY MEDICAL
    PROFESSIONALS IN THE EVENT OF AN EMERGENCY DURING THE BAJA RALLY SCHOOL.
10) UTV Applicants must have their co-driver/ navigator complete this form individually, for themselves and attest to doing so. If you are applying to attend the UTV RALLY SCHOOL as a solo driver, you must attest to having the automatic road book reader and navigation equipment to enter an actual rally race.
11) Your application is not considered accepted until the applicant has received an email confirmation from school@bajarallymoto.com
12) Non-resident Students expected to be checked into San Nicolas Hotel on Thursday night for casual reception and RALLY COMP overview. Shared hotel room provided by BAJA RALLY.
