Several of you have requested a contact list for birthdays and playdates.  I am going to put together a list of contact information to send home, but  can only share contact information I have been given permission to release.  If you would like to be included on the the contact list please fill out the form below with any contact information you would like to have shared such as your phone #, e-mail address, etc.  I will only include the information you include on this form.  If you do not want your contact information shared please indicate that as well.
