Please read and complete this form if you wish to participate in the "For the Love of Pocking" Tournament activities at Four Walls Squash and Social Club on Sunday February 21st at 1 PM (sign in at 12:30 PM).

In exchange for participation in the Indoor Pocking event being organized by the Bahamas Pocking League and held at the Four Walls Squash Club on Village Road, Nassau, Bahamas, I, the undersigned agree to the following.

1. I agree to obey and follow all posted signs, rules and warnings, and further agree to follow any oral instructions or directions given by Bahamas Pocking League referees (Listed below) or employees, representatives or agents of Four Walls Squash Club staff.

2. I recognize there are certain inherent risks associated with the above described activity and I assume full responsibility for personal injury to myself and (if applicable) my family members, and further release and discharge Bahamas Pocking League and Four Walls Squash Club for any injury, loss or damage arising out of my or my family's presence at or use of the facilities of Four Walls Squash Club, Whether caused by myself, my family, Bahamas Pocking League, Four Walls Squash Club or other third parties.

3. I agree to indemnify and defend Bahamas Pocking League and Four Walls Squash Club against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees, and other litigation costs which may in any way arise from my or my family’s use of or presence at the facilities of the Four Walls Squash Club.

4. I agree to pay for any damages to the facilities of Four Walls Squash Club caused by any reckless, or willful actions by me or my family.

5. Any legal or equitable claim that may arise from participation in the above shall be resolved under Bahamian Law.

6. I agree and acknowledge that I am under no pressure or duress to sign this agreement and have been given a reasonable opportunity to review it before signing. I further agree and acknowledge that I am free to have my own legal counsel review this agreement if I so desire.

7. I have read this document and understand it. By entering my name and email address below, I understand that I voluntarily surrender certain legal rights.
