We require DMYS students to be a part of their school orchestra or band program.

This waiver is for students who in good faith have tried to participate in the music program but have irreconcilable schedule conflicts that prohibit participation. In those instances, with this waiver and approval, students may still participate in DMYS.

This form must be completed by a school administrator (not the student or parent).
