PAR Materials Order Form
Hello PAR Volunteers!

Please use this sheet to request PAR materials and resources for your tutoring sessions. Requests will be fulfilled within 1-2 weeks from submission. You will receive a confirmation email from PAR staff when your order is shipped. Continually check your email from PAR staff in the event that we cannot fulfill your order or if substitutions need to be made. If making multiple orders for multiple learners, fill out this form once per learner.
