CONSENT TO DISCLOSE INFORMATION. I hereby grant my permission to JUNGLE RAPIDS or their
representative to investigate my previous employment, education background, character references,
and medical history. I release all persons who furnish such information to JUNGLE RAPIDS or their
representative from all liabilities and damages. I acknowledge and understand that I have hereby
received notices with the Fair Credit Reporting Ace. JUNGLE RAPIDS may seek to procure information
regarding my character, general reputation, person characteristics, or mode of living from consumer
reporting agency. I understand that upon my written request, JUNGLE RAPIDS shall make a complete
and accurate disclosure of the nature and scope of such investigation if one is made. Where legally
permissive, I also agree to submit to polygraph and drug tests or other similar monitoring methods at
the expense of JUNGLE RAPIDS. I understand that the facts discovered by such polygraph tests, drug
tests, or other similar methods may be used as evidence in legal or administrative proceedings and they
may also be used in considering my application or continued employment and as a basis of rejecting my
application or terminating my employment. I agree not to discuss my rate of pay or the rate of pay of
others with JUNGLE RAPIDS Employees.
I understand that misrepresentations, omissions of facts, or incomplete information requested in this
application may remove me from further consideration for employment. In addition, if employed, and
misrepresentations or omissions of facts called for in this application will be cause for dismissal at any
time without any previous notice.
IN CONSIDERATION OF MY EMPLOYMENT, I agree to conform to the rules and regulations of JUNGLE
RAPIDS and understand that I can leave the Company with or without cause and JUNGLE RAPIDS
reserves the same rights. I understand that no manager or any Company representative has authority to
enter into any agreement to the contrary. I also agree to the full release of liability to JUNGLE RAPIDS
for providing reference to all potential employers in the event of my termination. I understand that
JUNGLE RAPIDS is a tourist driven, yet local-based fun park, which requires its employees to be available
on the weekends and during the week. I also recognize that my appearance is crucial to maintaining the
already established dress code and clean family environment, THEREFORE, I agree to (1) remove all
excessive body piercings before work, (2) remain clean-shaven, (3) wear no more than 2 rings per hand
and keep all necklaces tucked inside of my shirt, (4) wear a name tag and keep my shirt tucked in when
on duty, (5) follow the established dress code of wrinkle-free khaki shorts or pants, a belt, socks, closed-
toed shoes and Jungle Rapids T-shirt, (6) leave my cell phone in my locker or car, so as not to be distracted while on duty.
