I. I agree to follow all terms and conditions listed in the Captivation Dance Policy & Procedures 2023-24 which can be on the website. If you cannot find them, please request them by emailing info@captivation-dance.com.

II. I understand that tuition is due on the 1st day of classes in each calendar month during the fall semester  (September-December) and the spring semester (January-June). 

III. I understand that there will be a set amount of payments based on my payment plan that will be made during the 2023-24 dance season; beginning September 1, 2023 and the last ending June 1, 2024. If I do not pay by the 7th of the month, I agree to pay a late fee of $35 and understand that after my account is 14 days past due, my dancer will not participate in any class related activities until the late fee & my tuition is paid in full.

IV. I understand that refunds are not given for any reason & if my student/ myself decides to drop a class, I must submit a written request by the 28th of the prior calendar month in which we are discontinuing dance. I agree to pay all charges to my account if the request is not submitted by the 28th. No refunds will be given for a student that is dismissed from recreational classes for any reason (ie attendance issues, dress code violations, behavior issues, etc). There are no makeup classes offered at this time.

V. I understand that the last day to submit a written drop request for the Spring Semester 2024 season is March 1, 2024, and if I do not submit a form, the dancer and/or parent must respectfully pay all outstanding and future tuition fees and costume fees that was agreed upon via signed contract, beginning the date of request through June 30, 2024.

VI. I understand that my account will be sent to a collection agency in the event that I do not pay any outstanding fees associated with tuition, late fees, class withdrawal, and/or abandonment of the studio without the completion of a written request by March 1, 2024.  I will be responsible for my balance due as well as any collection fees assessed by the collection agency.

VII. I understand that Captivation Dance, LLC provides equal opportunities to all participates and/or students without regard to unlawful consideration of or discrimination against race, religion, creed, color, nationality, sex, sexual orientation, gender identity, age, ancestry, physical or mental disability, medical condition or characteristics, or any other classification prohibited by applicable local, state or federal laws.

VIII. I understand that dancers attend Captivation Dance, LLC at their own risk & Captivation Dance, LLC, its members, staff and volunteers are not responsible for injury, loss, or theft at the studio or any premises as part of any dance class, performance or event. In the event of an injury and/or suspected injury, Captivation Dance, LLC its members, staff and volunteers reserve the right to sit and/or pull a student from dance class, performance or event associated with the studio. A note from a physician will be required for any injuries that may interfere with the dancers ability. This includes but is not limited to the foot, ankle, leg, hip, back, arm, neck, etc. Additional information is provided for the Competitive dancers in the Competitive Handbook.

IX. In the case of a medical emergency, I give Captivation Dance, LLC permission to treat my child with the appropriate actions. I understand that the primary phone number, email address and emergency contact will be notified immediately at the time of injury, if appropriate.

X. I understand that there is some inherent risk of my dancer being exposed to COVID-19 or other infectious disease despite all the precautions taken by Captivation Dance, LLC. As a result, you agree to absolve Captivation Dance, LLC of any liability related to the potential of contracting COVID-19 or other infectious disease.

XI. I understand that I am committing my dancer to the 2023-24 Fall and/or Spring Semester.  The amount for tuition and fees for the 2023-2024 season has been set regardless of the method of instruction and will not be refunded if the instruction occurs remotely or online for any part of the 2023-2024 year.
