APPLICATION FORM

DEADLINE: SUNDAY APRIL 3, 2022
Application for Practice Lab '22 requires submitting two forms:
1. Application Form (this form)
2. Audition Form (video links)
Acceptance notifications will be emailed on a rolling basis between March 1 and April 14 and will be sent within two weeks of receiving both the Application and Audition forms.
Once accepted, you will be invoiced for full tuition, due one week later.
All notifications will be sent by May 1, 2022
