This agreement releases E. L. DiGiammarino, Jr., Nordic Walking Instructor and QLM Nordic Walking division of  Quality Lifestyles Marketing, Inc., from all liability relating to injuries that may occur during Nordic Walking training and/or Nordic Walking events at any public or private location organized by E. L. DiGiammarino, Jr., Nordic Walking Instructor and QLM Nordic Walking division of  Quality Lifestyles Marketing, Inc.  

By checking "I agree" below, I agree to hold E. L. DiGiammarino, Jr., Nordic Walking Instructor and QLM Nordic Walking division of  Quality Lifestyles Marketing, Inc., entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.

I also acknowledge the risks involved in Nordic Walking in public locations.  These include but are not limited to tripping and/or falling.   I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

By clicking "I agree" below, I forfeit all right to bring a suit against E. L. DiGiammarino, Jr., Nordic Walking Instructor and QLM Nordic Walking division of  Quality Lifestyles Marketing, Inc., for any reason. In return, I will receive {participation in activity}. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.

Minors will need to have a parent present and sign a minor waiver at the first session.

I irrevocably authorize QLM Nordic Walking, a division of Quality Lifestyles Marketing, Inc. and the My SRQ Nordic Walking Friends group [Releasee] to copy, edit, enhance, crop, or otherwise alter any photo, or video, for use in their publications. I also waive any rights for approval or inspection of any photos or videos.. 

I understand and agree that all photos are the property of QLM Nordic Walking, a division of Quality Lifestyles Marketing, Inc. and the My SRQ Nordic Walking Friends group [Releasee] and will not be returned to me.

I acknowledge that I am not entitled to any compensation or royalties with respect to the use of the photos or videos.

I agree to release and forever discharge QLM Nordic Walking, a division of Quality Lifestyles Marketing, Inc. and the My SRQ Nordic Walking Friends group [Releasee] and its affiliates, successors and assigns, officers, employees, representatives, partners, agents and anyone claiming through them, in their individual and/or corporate capacities from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, causes of action of any nature or kind, known or unknown, which I, and anyone claiming on behalf of me, may have or claim to have against Releasee in connection with this Release.

I have carefully read and fully understand all the provisions of this Photo Release Form and am freely, knowingly and voluntarily signing.
