Back To School Schedule Pickup & Computer Distribution
Horizon is having an in person schedule pick up this year.  All of the documents/forms that are listed below are located on our Back To School page (https://www.d49.org/domain/1723). Students WILL NOT be able to receive a schedule without this form completed.  Students WILL NOT be able to check out a school computer (which is required) unless parents attend schedule pickup and sign the form for the computer.  If you have multiple students you MUST complete this form for each student.

We are always looking for parent representatives for our School Accountability Committee (SAC).  This committee meets 5x a year, and the administration team updates you on what is going on in the school and gathers input from you our parents!  Meetings my be held here at the school and/or on ZOOM.  If interested please include you name AND email below *
