LIHA members can use this form to anonymously contact the Board of Directors. Ask questions, share comments, make SafeSport notifications, etc.

Submitted forms will be reviewed and discussed monthly at the Board of Directors meetings.

Contact fields (name, email, phone) are not required unless you would like a direct response. Anonymous inquiries will be addressed in writing and posted to the LIHA website (after the monthly board meeting) at lincolnhockey.org/minutes.
