Please fill out a separate nomination form for each person - Teachers and Alumni that have already won this award are not currently allowed to be nominated again.

Teacher/Staff Member of the Semester:
1. Teacher must be a current USD #499 employee.
2. Teacher must have worked in the school district for a minimum of two years before being nominated.
Alumni of the Semester:
1. Alumni must have graduated from Galena High School.
2. Alumni must have contributed in some way to the Galena community and or exemplify what it means to be a Bulldog in their profession and/or the community in which they are living.
3. Alumni must have graduated at least five years prior to nomination.
