OMGHA Character Nomination Form

The Scholastic Achievement Ceremony will include a character component. Each travel team will nominate one player who best represents the traits promoted by the #11forJacob movement. The nominated player should be an excellent example of a child working hard to live these traits both on and off the ice, in the classroom, and at home.

The nominated player may meet the established scholastic requirements, but this is NOT required.

Individual head coaches may determine how the players are selected from their teams. The Jacob Wetterling Resource Center (JWRC) has a number of resources available for teams looking for ways to embed these traits within their team culture. Please reach out to us for support if needed.

Each head coach will fill out this form by the end of the winter season. Nominations are due no later than March 17, 2024
