I, "Participant", hereby WAIVE, RELEASE, DISCHARGE, FOREVER PROMISE NOT TO SUE The Friends of Georgia State Parks and its Chapters, Board members, officers, employees, and agents, as well as the State Tort Claims Trust Fund (the “Releases”) from and for any and all claims, demands, liabilities, causes of action, losses, costs or expenses, including attorneys’ fees, whether known or unknown arising out of or in any way related to my participation in the Activity. I, "Participant", understand, acknowledge, and agree that my participation in the Activity involves some risk of injury or death. I, "Participant" further understand that the Releasees assume no responsibility for the condition, operation, or safety of the Activity or the equipment associated with the Activity. I, "Participant", hereby KNOWINGLY AND FREELY ACCEPT AND ASSUME ALL SUCH RISKS and FULL RESPONSIBILITY FOR MY OWN SAFETY. I, "Participant" also agree to HOLD HARMLESS, DEFEND, AND INDEMNIFY The Friends of Georgia State Parks and its Chapters, Board members, officers, employees, and agents, as well as the State Tort Claims Trust Fund for any damage to, or injury or death of any persons (whether it be myself or another person) or property (belonging to myself or another person) or any other loss or damage whatsoever caused by me in connection with the Activity.

I grant Friends and its agents or employees the right to use photographs and/or video and audio taken of me. These images may be used in educational and documentary materials such as Public Service Announcements, Grant Applications, Video Documentaries and both printed and online newsletters.

Furthermore, I authorize the use of my image, likeness and voice for all program promotion, materials and any other purposes in connection with the program deemed appropriate and necessary by Friends including but not limited to advertising and marketing materials. I hereby agree to release, defend, and hold harmless Friends and its agents and employees, including any firm publishing and/or distributing the finished product in whole or in part, whether on paper, via electronic media, or on the use of the photographs/video, including but not limited to any misuse, distortion, blurring, alteration, optical illusion, or use in composite form, either intentionally or otherwise that may occur or be produced in taking processing, reduction, or production of the finished product, its publication or distribution. I also waive any right I may have to inspect and/or approve the finished product of advertising copy that may be used in connection with my volunteer services.

I hereby also agree to the above for my minor child(ren). I waive all claims for compensation for damages. I have read the release before signing below, fully understanding the contents, meaning, and impact of this release.
