Names on rugby shirts – application ユニフォームシャツお名前プリントお申込フォーム

From the 2019 Autumn season, we would like to add names to all rugby shirts in order to help coaching and communication with teammates.

We request that all current members submit their so that we can arrange for their name or nickname to be printed.

Cost of printing is 1,500 yen.
We will send you an invoice via Square. Please pay upon receipt.

2019年秋シーズンより、ユニフォームシャツのお背中にお子様のファーストネームまたはニックネームを入れさせて頂くこととなりました。つきましてはすでにユニフォームをお持ちの会員の皆様にも、プリント代１枚1,500円にてお名前の挿入を承らせて頂きます。チームメイトやコーチとのコミュニケーション向上のため、会員の皆様全員にご協力頂けましたら幸いです。下記よりぜひお申し込みをお願いいたします。

費用につきましては、後日経理担当よりスクエアを通して請求書をお送りさせて頂きますので、ご請求メールが届きましたらお支払いのお手続きの程よろしくお願い申し上げます。

(2)You can enter up to a maximum of 20 letters (please note that longer names will appear smaller).
(3)Indicate if you are completing one form for siblings by adding a forward slash between their names (“/”)

②最大２０文字までお入れできますが、文字数が多くなりますと1つの文字サイズが小さくなります。
③ご兄弟ご姉妹は「/（スラッシュ）」で区切ってご記入ください。

Notes regarding submission of shirt. ユニフォームシャツお預け時のご留意点

Please confirm you acknowledge the following points by checking the box below.

(1)The shirt needs to be washed before submitting.
(2)Write the name of your child on the tag inside the shirt using permanent marker (full name).
(3)Your child will need a shirt/t-shirt for practice until we can return the rugby shirt.
⑷ If you send your shirt to our club by delivery, we cannot be held responsible for any issues when posting. We suggest sending by recorded delivery.

①お名前をお入れするユニフォームシャツは必ずお洗濯頂いたものをお預けください。
②ユニフォームシャツの内側のタグに、油性ペンでお子様のフルネームをご記入ください。（プリントするお名前ではなく、フルネームでお願い致します）（添付画像をご参照ください）
③ユニフォームシャツお預かり当日、およびご返却までの期間は、私服のTシャツ等で練習にご参加ください。
④ご配送にてお持ちのシャツをお送りくださる場合は、ご配送時のトラブルにつきましては、クラブは責任を負えませんので、配達記録が分かる方法でご発送をお願い致します。
