Volunteer hours must be turned in within 30 days of service. This form is for PARENT volunteer hours only, do not submit student volunteer hours on this form. Any donations submitted for hours must be $10 or more and accompanied by a parent name and receipt. Only FSA requested/approved donations will be accepted for volunteer hours.
