Please be sure to fill this out if you would like to order matching spirit wear this season, even if you have previously emailed a spirit wear request in to us. One form per jersey/hat ordered please. If you would like to order more than one jersey/hat, please submit another form. An option to submit another response will be available upon completion of this form. Orders MUST be submitted no later than Monday, February 22 at midnight.  This form is ONLY for Assistant coaches and parent spirit wear orders. Head coaches order will be collected and ordered separate.

Coaches/Team moms: Park delivery/pickup date will be communicated once items are in. Please be sure to query your team to ensure you have received all payments via sealed envelopes prior to coming to pick up orders. All payment envelopes collected must have team/player's name written on front.
