Hello! We are excited to have you participate in Parent/Child Dedication on May 12 during the 10:00 service. This is always a special part of Mother's Day at Mt Carmel. Please fill in the following form to let us know that you plan to attend. We will send more details as the day gets closer. If you would like to dedicate multiple children on this day please fill out additional forms.
