The information provided needs to be able to be proven legally.  Only official names must be given, you will need to provide photo ID that should match.  
To complete this form you will need the subject specification document open for the exams you are wishing to make entries for.  Links available on see our series guide.

We require your UCI number, this is your Unique Candidate Identifier, and will be listed on any previous exam results you have gained.  If you have already sat national exams but cannot find this number, you can contact your previous school or college and they can give it to you.  If you have never sat any national exams, we can generate your UCI free of charge. If you have been issued a UCI number in the past and cannot find it, then there will be a fee of £5 for this service to generate a new one, you are expected to merge the UCI numbers.

You must be familiar with the subject specification for each subject, you are applying to sit an exam for.
