- Only one person can be nominated for each award type.

- A description of each award is displayed before the name and answer fields.

- You cannot nominate yourself.
- Type in the name of your nomination in the text field below each award type.
- Please describe why the person or organization deserves the award.
- A nomination entry for all reward types is not required.

- All replies are confidential. 

- We do ask for email addresses to verify if you are an IAD member to prevent spamming. 

- Once submitted - answers cannot be changed.
- You can request a copy of your responses by the email address you had provided.
- There is no need to sign into Google to save your entries - this is a short and sweet form.
- You may be required to select matching images prior to submitting to prevent robot submissions.
