If you were baptized at a different parish - we need a Baptismal Certificate from that parish.  Please call the Parish of Baptism and request one to be sent directly to St. Charles.
*We need to see a Certificate dated within 9 months of Communion Date to verify their Baptism. Baptismal Certificates are not kept on file from other parishes. If you turned in a certificate for another event you must must again submit to us your Baptismal Certificate (record) for confirmation unless baptized at St. Charles
